Email referrals

simplyendo is very happy to receive referrals via email and we have set up a dedicated email address just for this. Please send email referrals to

referrals@simplyendo.com

When completing your email, it would be very helpful if you could provide us with following information.

1) Patient's name, address, date of birth and contact telephone numbers.

2) Brief details of what you would like us to do for your patient, i.e. advice only or advice and treatment.

3) Details of the tooth/teeth involved together with an outline of the history of the present complaint and your provisional diagnosis (if known)

4) Details of any treatment already carried out.

It is very helpful for treatment planning if you can provide an up to date radiograph of the involved tooth/teeth. If sending us a referral via email, either digitally attach a radiograph or if you don't have this facility, please send the radiographs via post to our centre at

simplyendo, 70 Ellesmere Road, Altrincham, Cheshire, WA14 1JD

and importantly please make a note in your email that a radiograph is being forwarded under separate cover
